What’s happening when your jawline starts to soften and your smile lines deepen?

What’s really happening to your skin?

  • Skin quality matters: Over time, collagen and elastin decrease, resulting in skin that is thinner, less resilient, and more prone to sagging.
  • Fat pad redistribution: In youth, certain fat compartments (temple, lateral cheek, pyriform, etc.) are full and provide lift. With age and other factors (weight loss, medications), these may shrink, shift or descend.
  • Ligament and bone changes: The skeletal foundation stays (for the most part) but there is some resorption of bone in the jaw and cheek, and the ligaments that anchor soft tissue relax.
  • Volume loss: When support is lost in the temple and cheek area, gravity and soft tissue descent take over. That’s why you might see deeper smile lines, nasolabial folds, or jowls developing even if you haven’t “got older overnight.”
  • Skin laxity + volume loss = “droopy” look: When both processes are at work, the face may feel like it’s “falling.”

Can Botox (or any “tox” treatment) fix this?

No. Tox treatments like Botox, Dysport, Jeuveau, and Xeomin target muscle movement (dynamic wrinkles). They do not directly treat skin laxity or replace lost volume.
So if you’re seeing sagging, shifting, jowling, or volume loss—relying on “just tox” will not address the root causes. That’s why we talk about multiple modalities (i.e., multiple treatment types) at our med spa.
